Just enter your login information and update your information in the Members’ section of our website at www.cpso.on.ca 28 “We know physicians are busy. We think the ability to change address information online makes sense in this increasingly computerized world,” said Dr. Rocco Gerace, College Registrar. You can also send us a change of address form. At the back of each issue of Dialogue and on the College’s website, a change of address form is provided.  Please mail, fax, or email it to the College. You must notify the College within 30 days of an address change. The College’s register must contain your current mailing address, email address, and your primary practice address.  Your mailing address is the address you would prefer the College use to communicate with you and may be different from your practice address. It is NOT available to the public, unless you decide to use your primary practice address as your mailing address. Your primary practice address is available to the public.  The College also uses your email address for some communications.  Your email address is NOT available to the public.   Please note that, at press time, online address change was not yet available to first-year members or medicine professional corporations.
