What is a Community Health Worker?

Community Health Workers bridge the gap between individuals and the healthcare and social services systems. CHWs represent the needs and interests of their clients.

What do CHWs do?

•             Help clients navigate health and social services

•             Help people self-manage their health conditions through health education and goal- setting

•             Help clients communicate with health care providers

•             Access community-based services such as transportation, food assistance, and other programs
