some mild swelling of her lower lip
without any other abnormalities.
A custom profile was submitted to
check her CBC, chemistry, FeLV/FIV,
T4/FT4 and UA. Willow was
discharged with instructions for the
owner to give diphenhydramine
suspension (5 mg or 2 ml) by
mouth every 8-12 hours to see if
the mild swelling in her lip resolved
as I was suspicious of an allergic
reaction.
Her CBC was normal, with the
exception of a mild lymphocytosis
(6.9 K/uL). Her chemistry was
normal with the exception of a
mildly decreased ALP (14 U/L) and
AST (8 U/L) which was not clinically
significant. Her T4 was low at 0.7
ug/dL (normal 0.8-4.7 ug/dL), FT4
ng/dL was <0.3 and FT4 pmol/L
was <3.9 (reference range T4/FT4 1
-4 ng/dL). Her FeLV and FIV were
negative. A diagnosis of congenital
hypothyroidism was made based
upon the low thyroid hormone
assays.
Willow was started on
0.05mg of Levothyroxine orally
once daily with plans to recheck
her thyroid levels in 4 weeks.
Willow came back in for a recheck
4 weeks later. Her weight was 5.53
pounds. Her bradycardia and
gingivitis had resolved. There was
some eruption of her permanent
teeth. There was also some profile
development of her face. Her nose
began to lengthen and her cheek
bone structure was more defined.
The owner reported that Willow was
acting like a completely different cat.
She was more active, playing with
her littermate and no longer finding
the warmest place to sleep. The
owner didn’t realize Willow was so
lethargic and heat seeking until she
was no longer exhibiting these
behaviors. A blood sample was
obtained, 4-6 hours after her
morning dose of the Levothyroxine
and submitted for a repeat thyroid
profile. Her T4 was 1.2 ug/dL, FT4
ng/dL was still <0.3 and her FT4
pmol/L was still <3.9 (reference
range for T4/FT4 is 1-4 ng/dL). The
owner was instructed to increase the
Levothyroxine to 0.1 mg orally once
daily. The owner was to return in 6-8
weeks for a recheck.
Willow came back in 12 weeks for a
recheck. She reported that Willow
seemed even more energetic than
before. She weighed 7.97 pounds
