Student Contract

This contract / agreement is made between the Enfield Adult Education program and the student listed below . In consideration of the agreements contained here , both parties agree to the following :
Fees : The cost per student for the certified Nurse-Aide Training Program ( CNATP ) will be payable to Town of Enfield at registration . Fees include use of equipment , name tags , and a textbook . Fees do not include the cost of the State License and Registration Examination which is required for future employment as a CNA .
Student Selection : Students must submit proof of negative Tuberculin Testing PPD / negative . In addition , students must submit proof of a physical examination within the past year and have no medical condition that would preclude ability to provide care for residents including lifting , transferring , and ambulation . Students must also be free from communicable diseases . Students must have a seasonal flu shot , as well as the COVID vaccine . All personal medical expenses are the responsibility of the student . Students selected for the CNATP will be required to sign this contract . Registration will be conducted on a first-come , first-served basis without regard to race , religion , color , sex , age , or national origin .
Attendance : Because of state regulations , attendance is mandatory for all classroom and clinical work . The school reserves the right to withhold certification of students based on attendance records in order to be in compliance with State regulations .
Substance Abuse : A student who is on school grounds during a school session or anywhere at a school-sponsored activity , including clinical , is under the influence of alcohol , narcotics , restricted drugs , mood altering substances , or any substance purported to be a restricted substance or over the counter drug , shall be subjected to discipline up to and including dismissal from the CNA program . Students may be subjected to drug / alcohol assessment when entering or during the CNA program .
Flu Shot / COVID Vaccine : Students must provide proof of annual flu shot , as well as proof of COVID vaccination status . According to most healthcare facilities , having either 2 doses of the Moderna or Pfizer vaccine or 1 dose of the Johnson & Johnson as well as a booster ( after 6 months after receiving the second dose ) is required to work in the state of Connecticut and complete your clinical rotation .
Dress Code : Student dress for class and clinical will consist of white scrub top , black scrub pants , white sneakers / shoes . A wrist watch with a sweep-second-hand to allow for pulse and respirations to be taken , is required . Hair will be worn off the shoulders either tied back or put up . No leggings / stretch pants or low cut blouses . No open toe shoes or sandals will be worn . Nails need to be kept short throughout the program . Keep Jewelry to a minimum , no hoop or dangling earrings . Name tags and uniforms will be worn for all clinical work . Uniform coast is the responsibility of the student . The uniform is an important part of the CNA role for safety and professionalism . Students are expected to comply with the uniform protocol at all times .
Grading : Students must maintain a seventy percent ( 70 %) average to remain in the program . The faculty reserves the right to withdraw students for excessive absence from class or for unsatisfactory achievement of classroom or clinical objectives . Grading will be discussed further at orientation .
Class Cancellation : In the event of inclement weather or other problems which necessitate class cancellation , call the Enfield Public School closing hotline at 860-253-5170 for up-to-date information . Cancellations may also be listed on the following television and radio stations : WTIC Radio 1080 AM Hartford and WFSB Channel 3 ( listen for Enfield Public Schools on the list of postponements and cancellations ). In the event that you will be absent from class , please call 860-763-7032 and leave your name , phone number , and a brief message for Instructor . In the event that you will be absent from the clinical experience , you MUST call your clinical instructor at the number provided at orientation .
Smoking : Smoking is prohibited inside the classroom buildings where classes are held and at all clinical location sites .
Meals : We suggest that you always bring your own brown bag lunch for class and clinical sessions .
Phone Use : Please refrain from phone use during class time .
Parking : Students will be notified of designated parking areas both at the school and at the clinical sites . Students are advised to lock their vehicles for security reasons . Also , students are advised not to bring handbags or purses to the nursing home during their clinical training .
