Collaboration is the Future
Under the Delivery System Reform Incentive Payment System (DSRIP), new models
are being created that will result in larger collaborations among hospital systems and
related community services – including hospice and palliative care. With changing
models of care and services delivery in mind, a recent LeadingAge New York Joint
Board Retreat concluded that new partnerships and collaborations are needed that will
strengthen member’s ability to provide important care to New York’s elderly. This issue of
LeadingAge New York Adviser features stories that exemplify LeadingAge’s commitment
to collaboration.
LeadingAge New York and the Hospice and Palliative Care Association of New York
