Additionally, by virtue of
the demographic of New
York’s MLTC population
and general lack of
universal design in New
York’s housing inventory,
it was concluded that
the majority of managed
care plan members could
benefit from basic home
modifications aimed at
falls prevention.
After 18 months of network development, a partnership with the
USC Gerontology Department’s executive education program
for home modification contractors, another partnership with the
Home Depot, and some technology development, Evolve Emod
was born. The goal was an organized delivery system for
home modifications designed to allow for managed care

bids, coordinate home modifications, measure quality and

This is but one example of creative destruction. Some might
simply call it innovation but NMN and Evolve Emod want to
