C) One dose of the updated Pfizer, Moderna or Novavax vaccines, administered September 2023 or later
16 Influenza Vaccine( based on annual flu season) Due when available and before October 1
Influenza vaccine documentation must include lot number and expiration date for the vaccine
17 Students are required to submit a headshot style photo. Photo must be on a white or lightcolored background and well lit( similar to a passport photo). Professional attire or scrubs required.
18 Government issued ID: Students must upload a photo of the front and back of a driver’ s license or government issued ID. Acceptable forms include passports, driver’ s license or government ID.
