Medical Plan Options
Renown Health offers three medical plans to choose from:
Standard Plan
The Standard Plan has qualities of both HMO and PPO plans, with benefit levels dependent on where the
participant receives care.
Primary care providers will coordinate all aspects of care. Participants receive the highest level of benefits
by using Renown providers and services. For most services provided by Hometown Health (HTH) preferred
providers pay co-insurance (percentage of cost). Services provided by non-preferred providers are covered
at a significantly reduced level.
Employees with dependents outside of the HTH service area have access to the national network of
preferred providers. Add your dependents’ out-of-area address during open enrollment or notify the
benefits team throughout the remainder of the year.
Enhanced Plan
The Enhanced Plan is a health maintenance organization (HMO).
Primary care providers will coordinate all aspects of care. NEW THIS YEAR – two levels of benefits.
Participants receive the highest level of benefits by using Renown providers and services. Services provided
by Hometown Health (HTH) preferred providers pay higher copays. There is no coverage for services
provided by a non-preferred provider except in emergencies.
Limited Plan
This plan is 100 percent employer-paid if the participant and participating spouse achieve the highest level
wellness discount. It includes higher co-pays for Renown services than the other plans and high deductibles
and coinsurance for HTH preferred providers. This plan provides limited prescription drug benefits and no
vision benefits. Eligibility is limited to full-time employees and eligible dependents. Services provided by
non-preferred provider are covered at a significantly reduced level.
Hometown Health administers the medical, dental and prescription drug plans.
