When Your Coverage Begins (Effective Date) Coverage starts on the first of the month following 30 days of consecutive employment in a full- or part-time position. When and How Do I Enroll? Original Enrollment Period To complete the online enrollment, go to Kronos > Employee Home Page > Benefits > Life Events within 30 days of your hire date. If enrollment is not completed timely, you will be enrolled in the Standard Medical/Standard Rx and Vision plan for yourself only. The necessary deduction will be deducted from your paycheck. Changes cannot be made until the next open enrollment period unless you experience a qualifying life event. Open Enrollment (November 1 − 30) This is your opportunity to choose your benefits for the following calendar year. If no changes are made during this period, elections for the current year will continue for the next calendar year. However, you must re-enroll in Flexible Spending Accounts every year and, if covering a spouse on your medical plan, you must complete the Spouse Coverage Affidavit yearly. Making Changes After Open Enrollment Qualifying Life Event If you experience a qualifying life event, complete the appropriate Life Events online enrollment in Kronos within 30 days. Proof of the qualifying life event must also be provided to HR within 30 days. Changes must be consistent with the type of qualifying life event. Qualifying Life Events • Marriage • Divorce • Birth or adoption of child • Death of dependent • Change of the employment status of you or your spouse that results in a loss (or gain) of coverage • You, your spouse or dependent child becomes entitled to Medicare, Medicaid or COBRA coverage under the plan • A change from full-time to part-time employment (or vice versa) • You or your covered dependents moving into or out of an area that affects your coverage Notify Human Resources Within 30 Days You must notify HR of the qualifying event by completing the appropriate Life Event link in Kronos on your Employee Home Page under the Benefits tab and providing supporting documentation. Don’t Forget! If you don’t add your newborn to your insurance within 30 days, the baby won’t have coverage until the next year. 29
