Student’s Transcript or Recent Report Card – is needed for new and returning students to place the student in the proper grade. This applies only for grades 2-12. Student Immunization Records – are required before a student can enroll into school. Immunization and medical records can be reviewed by Student Assignment Center staff or at the school of assignment. > Step 2 – Fill out Forms Pick up Form Blank forms both in white (for K-12) and blue (for Transitional Kindergarten) are available at the Student Assignment Center or at any school. If you have any questions regarding the appropriate form to complete, talk to your school representative or Student Assignment Center placement specialist for more information.
