“c/o or “in care of” are accepted)
• both automobile registration and auto insurance policy
(must be current and will count as one proof)
• a homeowner’s or renter’s insurance policy.
• A lease agreement with the landlord’s contact information.
• A property tax statement from the current tax year, Grant
deed/Title, or if recently purchasing a close of escrow proof
or mortgage documentation.
• Official government correspondence with the parent/
guardian (IRS, Alameda County, California EDD, Social
Security).
OUSD reserves the right to request additional proof of
residency and investigate any claim as deemed necessary.
Student’s Transcript or Recent Report Card – is needed for
new and returning students to place the student in the proper
grade. This applies only for grades 2-12.
Student Immunization Records – are required before a
student can enroll into school. Immunization and medical
records can be reviewed by Student Assignment Center staff
or at the school of assignment.

Blank forms both in white (for K-12) and blue (for Transitional
Kindergarten) are available at the Student Assignment
Center or at any school. If you have any questions regarding
the appropriate form to complete, talk to your school
representative or Student Assignment Center placement
specialist for more information.

Submitting the Form
The application form and required documentation must be
submitted in person by the parent or guardian of the student.
All original documentation must be submitted with the form
where it will be copied and attached to the application. This is
to ensure that the student’s proof of residency, guardianship
and age has been received by the required due date.
Parents submitting a pre-printed ap
