To wear it, you may be any school and a minimum level of 52. Wearing it gives you the bonus of 196 extra max health and a dark pact item card. This amulet is not dropped or sold (just the amulet without the effort of crafting) by anyone, so the only way to get this necklace is to buy the recipe and craft it. It is also No Auction.
