RETURN FORM TO STUDENT AFFAIRS OFFICE
DOCUMENTATION UPON RETURN
For Interviews Interview Requests should be submitted 3 days in advance with supporting documentation of the interview from the prospective employer . Students may not request leave for interviews during finals . However , in certain situations , the Vice President of Academic Affairs / CAO is authorized to make special exceptions . This process also applies to any phone or virtual interview that may conflict with lineup or any class or shop period .
For Medical Appointments for routine medical care should be made during breaks , days off , and evenings . Students must understand that they , not their parents , are responsible to comply with college policy in this matter .
Students are required to submit medical documentation to the Student Affairs office upon return , verifying the appointment was kept as scheduled .
For Athletics Injuries Students are required to submit medical documentation to the Student Affairs office upon return , verifying the appointment was kept as scheduled .
For After Hours ( emergencies only ) Students should report the issue to their Dorm Manager or the Director of Residence Life in their absence . The Director must authorize the departure ( in extreme circumstances , this step can be bypassed , according to the Manager ’ s judgment of the situation ). Return the form to the Manager for submission . Students will be asked to provide documentation / explanation regarding their request .
In addition , students who depart campus in the evening are expected to return as required by the terms of their specific leave request . Illness is NOT a valid excuse to not return to campus ; students who leave and attempt to call the Attendance Line to excuse such an absence are considered AWOL .
Privilege to Leave Campus in the Evening During the week , students may only leave campus by submitting for an approved absence as noted above , OR based on their class year and performance :
• Seniors may leave campus M-Th after Evening Lineup but must return by 10pm .
• Juniors can also earn leave privileges based on previous semester GPA ( see “ Senior Privilege for Juniors ”).
• All students are eligible to earn Overnight Leaves as noted under “ System of Rewards / Incentives .”
• Freshmen and students on Academic or Disciplinary Probation may not leave campus in the evenings during the week .
• Students may not leave campus during lunch without written permission from Student Affairs .
