presentations . A pulmonary nodule causes architectural disruption , which can lead to a pneumothorax . In our case , histoplasmosis was the cause of the nodule . Therefore , it becomes important for clinicians to consider a nodule ’ s positioning in the lung when pondering possible clinical implications , regardless of its exact etiology . Our case also highlights the importance of an algorithmic approach to the management of an SPN . Lastly , treatment of histoplasmosis is outlined in current IDSA guidelines , but as in this case , directed antifungal therapy is not always necessary .
