ALLOTMENT INFORMATION
Washington State Dairy Council offers one $20.00 complimentary allotment (credit) per qualified person each
calendar year towards purchasing any of our materials. Orders received after December 14th, 2014 will be
applied to 2015 allotment. Warehouse closed Dec. 15, 2014 - Jan. 2, 2015.
PLEASE READ THE FOLLOWING CAREFULLY.
• Shipping and tax charges do not apply to allotment orders of $20.00 or less.
• $20.00 allotment may not be split into two or more orders. Total allotment amount must be ordered at one time or you will forfeit your remaining balance.
• Only residents of Washington State and Alaska who are directly involved in teaching nutrition and fit within the qualified titles are eligible for this allotment.
• QUALIFIED TITLES: PE/Health Educators, K-12 Teachers, School Nurses, Food Service Directors or Managers, WIC and Public Health Nutrition Educators,
Physicians, Dietitians and Nutritionists, Worksite Wellness Coordinators, Nurses, WSU and UAF Extension Professionals.

PAYMENT INFORMATION
• Prices are current as of January 1, 2014, and are valid through December 31, 2014. Prices are subject to change.
• Delivery Time will be approximately two to four weeks.
• Orders Outside of the United States must be pre-paid by check or money order in U.S. currency and
drawn on a U.S. bank. Visa, MasterCard or American Express are accepted.
