I, __________________________________________, understand and agree to be solely responsible for the above financial commitment (tuition, team
fees). Team items will NOT be distributed if payments are past due.
The following payments are NOT pre-rated: Miscellaneous fees, Registration Fee, and Team Fees.
Tuition Fees and Team Fees are NON-REFUNDABLE including cases of illness/injury, joining late, or leaving early. If my child leaves the program before
April 30th, 2016, I forfeit all TUITION PAYMENTS and will be charged a $250 Breaking Contract Fee. All TEAM FEES must be paid in addition to the $250
Breaking Contract Fee. Pay-in-Full prices are discounted and therefore are 100% non-refundable.
Payment tardiness will result in late fees, and my child possibly being sat-out of practice and replaced for competition until balance is paid.
Travel expenses are at the responsibility of each family. I understand the National out of state competition fees are NOT INCLUDED in the above amount!
Absences must be approved through the gym.
Practices one week prior to a competition and two weeks prior to a National Competition are mandatory. If my athlete misses for any of those mandatory
practices, I understand that they may be replaced at the competition.
There will be a $20 fee added to my tuition if I do not put my financial information on file for monthly tuition withdraw.
