Membership
Section 1: Membership Qualifications
Membership in the USS Niagara shall be open to anyone who wishes to join and fills out
an application, regardless of race, color, sex, religion, age, sexual orientation, disability, or
veteran’s status, provided they maintain a good moral character, pass a basic background check
(including the Ship’s commanding officer (or designee) speaking to the member’s prior ship, if
applicable), and abide by the rules set forth herein.
The member shall be required to provide specific information (as provided for on the
Ship’s Application for Membership) or as may be required by the command crew of the ship.
None of the information provided shall be used in any manner which might embarrass any
current or prospective member of the crew and shall be maintained in accordance with all local,
state and federal regulations pertaining to the protection of an individual’s privacy. The
information collected shall be:
? full name
? full address, including city, state and zip code
? date of birth
? phone number
? e?mail address if one exists
? notice of specific interests and or current job, abilities or talents (this information is to
be used by the commanding officer, executive officer and first officer as means to
establish a crew assignment to a specific duty section within the Ship)
? emergency notification with phone number (in case you are injured and the Ship needs
to make notifications)
? medical history including allergies (this information shall be utilized only in the case of a
medical emergency and provided only to an emergency medical provider in case
treatment is required during a ships function)
This information shall be included in a sealed packet including a card for each member of
the crew and maintained by the chief medical officer.
These sealed packets shall be provided to the senior member of any away team when more
than 2 members are participating in any function, at the request of said senior member. The
information shall remain sealed and in the possession of the senior member, and shall not be
made available to any person who does not have a legitimate need to know that specific
information.
