Life Insurance Life insurance provides financial security for the people who depend on you . Your beneficiaries will receive a lump-sum payment if you die while employed by Unified Fire Authority . The company provides basic life insurance of $ 25,000 , up to age 70 ; $ 12,500 at age 71 to 76 , and $ 6,250 at age 76 or older . If you ’ re enrolled in basic coverage , you get an additional $ 50,000 line-of-duty death benefit at no extra cost . Enrollment is automatic .
Accidental Death & Dismemberment ( AD & D ) Insurance Accidental Death & Dismemberment ( AD & D ) insurance provides payment to you or your beneficiaries if you lose a limb or die in an accident . Unified Fire Authority provides AD & D coverage of $ 25,000 at no cost to you . Enrollment is automatic and this coverage is in addition to your company paid life insurance described above .

Voluntary Life Insurance

You may purchase life insurance in addition to the company-provided coverage . You may also purchase life insurance for your dependents if you purchase additional coverage for yourself . You are guaranteed coverage up to $ 200,000 , and up to $ 50,000 for your spouse without answering medical questions if you enroll when you are first eligible .
