Completed at the end of freshman year and prior to enrollment in sophomore-level nursing classes
Yearly based on academic year . Complete after June 1 and due by August 1 * every academic year a student is enrolled in a nursing program .
2 Medical History and Physical Examination
Student must submit all pages of Health form . Health form can be downloaded from the Complio website .
3 Proof of Health Insurance Coverage
Submit a copy of your Health Insurance Information ( card or plan details )
4 Urine Drug Screen

* Labeled Package 2 in Complio 5 Tuberculosis Screening
Initial tuberculosis screening MUST be completed via one of these two blood tests : Quantiferon-TB Gold or T- Spot
A tuberculin skin test ( PPD ) is not sufficient for this initial screening .
