To maintain compliance with FMMSNHP requirements and attend all clinical practice experiences , you must adhere to the initial submission deadline and maintain all yearly requirements prior to their expiration dates .
NOTE : BSN and WBSN - Students must complete all documents after June 1 and submit by August 15 of each year . ABSN – Students must have compliance by the start of their first semester and maintain compliance every semester thereafter .
Health documents must include the student ’ s name and healthcare provider information on each document .

Main Campus : Completed at the end of freshman year and prior to enrollment in sophomorelevel nursing classes ; complete after June 1 and due by August 15
WBSN : Completed prior to the start of the Fall Semester .
Yearly based on academic year . Complete after June 1 and due by August 15 every year a student is enrolled in a nursing program .
1 Medical History and Physical Examination and Proof of Health Insurance Coverage
Student must submit all pages of health form . Health form can be downloaded from the Complio website .
2 Proof of Health Insurance Coverage
Submit a copy of your Health Insurance Information ( card or plan details )
3 Urine Drug Screen Urine drug screen must be completed as directed through Complio within 30 days of registration Random drug screening may be required . A positive result in a drug screening will result in dismissal from the Nursing program .
4 Tuberculosis Screening
Initial TB screening : results of QuantiFERON-TB Gold ( blood test ) or T-Spot ( blood test ) A tuberculin skin test ( PPD ) is not sufficient for this initial screening .
ABSN : Completed prior to the start of the student ’ s first semester . X
