THE SUPPORTIVE CARE
CLINIC:
is an interdisciplinary
approach to caring for
patients with serious
illnesses, whether they are
in active treatment or have
completed treatment. The
goal of this program is to
help patients fulfill their
maximum physical, emotional,
spiritual, vocational, and social
potential. Appointments can
be made by self-referral or
physician referral.

THE BREAST CANCER
SURVIVORSHIP PROGRAM:
offers specialized, long-
term follow up care for
breast cancer patients after
treatments end. Participants
in this program are monitored
for any long-term effects of
breast cancer treatments,
referred for management
of identified problems, and
provided a Survivorship Care
Plan including a personalized
record of treatment,
guidelines for continued
monitoring, and information
on support resources.
Appointments can be made
by self-referral or physician

SERVICES:
assists with developing an
exercise training designed to
optimize health and wellbeing
in a way that helps cancer
patients transition into a safe
exercise program during and
after treatment. Services
offered include fitness
assessments, one-on-one
or group exercise sessions,
and education on safety and
