The Big Event 2013
Youth participants need to complete the camper registration form. Adult counselors, those age 21 and over, must complete the counselor registration form. Please make sure the following information is filled out correctly and complete.
! Health Information including insurance information, social security

number, and medications for campers and counselors. ! Roommate Selection—please select ONE roommate. We can’t squeeze a dozen closest friends into a cabin, but do our best to place churches together. ! T-Shirt Size—T-shirts are in ADULT sizes: S M L XL XXL. If we have to guess what size you want, you’ll get an XXL. ! Important Signatures—Youth Forms: Youth & Parent signatures Counselor Forms: Counselor signature and date and Senior Pastor signature and date. ! Medication—All medication, either prescription and/or over the counter must be in the original container or prescription packaging. Place it in a zip lock bag with the camper’s name on it. An Adult chaperone from your church will collect and administer medications. All adult sponsors who register must have a background check completed by their local church and received Safe Sanctuary training. Each registered participant, youth or adult, must pay the registration fee. ! Registration: The cost for the retreat is $100 per participant, if electronically registered and forms postmarked by October 4, 2013. The cost goes up to $125 per participant if electronically registered and postmarked by October 11, 2013. We will not be taking any registration forms AFTER October 11.
