The process for incident tracking was kept very simple . At each site , medical volunteers logged each spectator visit on a document provided by leadership . For any athlete or spectator medical incident with a disposition , a medical incident form was completed and sent to senior leadership .
• Lower-acuity spectator incidents did not require a medical incident form , such as handing out an adhesive bandage of treating a spectator for a headache .
• Medical incident forms were always filled out for athletes and higher acuity spectator encounters .
At the end of each event , volunteers scanned logs into a secure shared drive . These scanned documents were received by data entry staff , who created data entries that fed into a medical incident summary dashboard . This was a daily procedure that helped produce a comprehensive summary of incidents for all events .
