“ Your Black Swan is Someone Else ’ s Gray Rhino : Some Thought on sharing Institutional Knowledge ”

The program will begin with some brief comments on the “ cause ” of tragedies in any occupation or profession , focusing on the difference between “ proximate ” cause and the real problems “ lying in wait ,” sometimes for years , that went ignored and really led to the given tragedy . The attendees will be provided with strategies for recognizing the problems “ lying in wait ” so that they can be properly addressed prior to problems occurring . It will then focus on elements contained in the title – Black Swans – The Unknown Unknowns – and Gray Rhinos – massive problems coming right at us that we refuse to recognize and / or acknowledge . Too often the events that “ sneak up on organizations ” and cause tremendous problems have been experienced by other similarly situated organizations – but the uninformed people in the other organization were unaware of their existence and how to address the given risk proactively thus resulting in a preventable tragedy .
This will be followed with some brief comments on the “ Ten Families of Risk ” and give the attendees a “ take back to work ” assignment to help them get started on identifying and addressing the real risks faced in the various job descriptions in your specific organization – again by studying past tragedies .
Finally , Mr . Graham will demonstrate to the attendees some ways on how to better “ learn and share ” knowledge learned from similarly situated organizations . During this portion of the program Gordon will address the importance of developing final reports on tragedies in your organization , bringing back the best of the best employees to train , mentor and develop supervisors and managers , the importance of accurate investigations of tragedies , some thoughts on non-punitive close call reporting and some techniques to capture information from great employees prior to retirement , and some thoughts on the importance of daily training on lessons learned .
