Pre-Empting Asthma Triggers

barriers and exposures to asthma triggers that prevent families from effectively
treating asthma during a clinic visit.

That’s why the Children’s Hospital Colorado asthma team designed the Just Keep
Breathing (JKB) asthma home visit program to supplement asthma specialist clinic
visits. JKB community health workers visit homes to assess triggers and educate the
family. They also help patients and families identify barriers to treating asthma and
help the family create a self-management plan.
“We work with families to understand how their environment, understanding
of asthma, and home life affect kids’ breathing,” says Monica Federico, MD, lead
physician for JKB. “If we can understand that, then I think we can do a better job of
improving their health.”
Included in Just Keep Breathing:
• Three to five home visits
• Asthma education specific to each family
• Patient assistance to help overcome barriers to care
• Home walk-through to look for triggers
• Limited home remediation to remove triggers, including
cleaning supplies
• Support from a community health worker, nurse and
social worker

Children who have seen an asthma specialist at Children’s Colorado and have had
an emergency department visit or hospitalization due to asthma in the past year are
eligible. The program is free for residents within 20 miles of Children’s Colorado’s
location in Aurora, Colorado.
