Tailored meaningful activities Help to feel better and reduce burdens Peer support & networking Small group sessions for personalised support Skill development sessions to strengthen at-home care abilities Information sessions to improve the carer ’ s knowledge of dementia care Referrals to relevant services Help with identifying the carer ’ s goals and needs through personalised assessment and care planning .
