INFORMATION EXCHANGE # 1 OF 2013:
“Accommodation Ladder Wire”
All, Recently we had an accident of snapping of accommodation ladder wire on board one of our vessel. This is a serious near accident which could have led to fatality of personnel. As you can see from the pictures it is obvious that lack of maintenance is the root cause. Root cause: Accommodation ladder wire was not maintained adequately.

Initial corrective action: All vessels to carry out PMS work order which shall be showing due on 31 st March 2013. This is included by PMS administrator for one time only. Job consists of below items. Remove accommodation ladders wires. Clean the wires thoroughly. Examine condition thoroughly. Re-grease. Report last renewal date of accommodation ladder wires.

Action to avoid re-occurrence - PMS system is already updated with below periodic planned jobs. Jobs are scheduled for 3 and 12 months interval. 1. Ensure to inspect the Accommodation ladder wires at 3 months interval. Inspection to be carried out by a senior officer. 2. Greasing of the wires to be carried out same time when wires are removed for inspection. 3. All sheaves and other moving parts shall be inspected and greased at 3 months interval. 4. Confirmation of this inspection is to be sent to vessel superintendent and to be entered into the Planned Maintenance System. 5. Accommodation ladder wires to be renewed annually.
