MISSING GLOVES – EXPOSES ENGINEER
WHAT HAPPENED -
After dismantling no. 1 Fuel Oil pump, the rotor pulley was dirty with
FO so it was being cleaning with rags, during the cleaning process the sharp edge of the rotor pulley struck the
left thumb of Gas Engineer resulting in a minor cut in the finger, Gas Engineer report about that to 1st Officer.
First aid was administered immediately.
WHY DID IT HAPPEN –
The Gas Engineer removed his gloves momentarily so they
would not get soaked in dirty oil and overlooked his personal safety in the process.
LESSON LEARNED –
Never under-estimate the use of PPE, even a
moment without it, leaves you vulnerable. The co-workers have to ensure that
those working around are Properly Protected Every-time.
SLIPPERY STEPS SLAM SPEEDING SEAMAN
WHAT HAPPENED -
While he was going to Engine Control Room for attending an Alarm.
4th Eng-Slipped on the stairway between A-Deck and upper deck on the wet surface of
deck floor. During the slip the side railing of the stairwell hit his chest thereby causing a minor bruise injury.

WHY DID IT HAPPEN –
The floor plates had just been mopped by motorman, who
failed to put appropriate physical barriers or warning signs in place. The 4th Engineer was in a hurry to put off
a screeching siren and was not careful and going too fast without holding onto the rails while descending.
LESSON LEARNED –
Speed Kills – whether you are at work or on the
road. No job is so urgent that it should not be done safely.
Physical barriers
must be put in place where there is a risk to someone getting hurt.
BOSUN BREAKS BONES BY BEING BRAVE
WHAT HAPPENED -
THE LASHING HAD TO BE TAKEN ON
THE PILOT LADDER REEL PLATFORM ON THE PORT SIDE OF THE VESSEL
AS IT WAS NOTECED THAT IF THE WATER CAME ON THE PLATFORM
COULD GET DISLODGED FROM POSITION, THE BRIDGE WAS ASKED TO
ALTER THE COURSE TO THE WAVES WOULD NOT BREAK ON THE MAIN
DECK AFTER CONFIRMING THAT THE VESSEL HAD ALTERED COURSE,
CREW MEMBERS BOSUN AND 2NOS OS WERE SENT AND TAKING LASHING
OF THE PORT GANGWAY PILOT LADDER REEL WHEN A WAVE WASHED ON
DECK TO MAKE THEM LOOSE THEIR FOOTING AND HIT THE DECK , THE
BOSUN'S ARM WAS NOTICED TO BE BROKEN BELOW THE ELBOW AND THE
OS HAD A SWELLING ON RIGHT SIDE OF THE FACE ON THE CHEEK.
IMMEDIATLY THE BOSUN AND OS WERE EVACUATED FROM THE DECK
AND BROUGHT INSIDE WHERE MEDICAL ATTENTION WAS GIVEN TO
THEM SPLINT APPLIED AND PAIN KILLERS ADMINISTERED.
WHY DID IT HAPPEN –
THE MASTER MAY HAVE
MISJUDGED OR UNDER-ESTIMATED THE FORCE OF WATER HITTING THE
CREW DURING THEIR BRIEF SOJOURN ON DECK. THE EXPERIENCED CREW
SHOULD NOT HAVE EXPOSED THEMSELVES TO THIS EXERCISE.

PREROGATIVE. EACH ONE OF US ONBOARD OR AHSORE HAVE
TO ANALYSE THE RISKS TO US AND QUESTION ANY TASKS
WHICH MAY PUT US TO RISK. A TRUE SAFETY CULTURE WILL
COME ABOUT WHEN EACH OF US ASSUMES RESPONSIBILITY
FOR OUR OWN SAFETY. A RISK ANALYSIS, FOLLOWED BY
PROPER
RISK
MANAGEMENT
/
MITIGATION
COULD
HAVE
PREVENTED THIS INCIDENT.
