INJURY – FACE
S. No. 100 Date 04-Jul-2013 Occurrence 100 - one tanker was en route towards Fujairah and deck crew was performing deck maintenance. one OS had completed chipping and changed his jet chisel to a grinder to smoothen deck surface. While changing the jet chisel, sudden burst of air occurred at the air hose quick connect coupler and generates rust to penetrate the eye goggle and embedded in his right eye. Eye wash was immediately applied by ship’s medical officer. Some of the rusts were removed but small particles remained in the eye causing some bloodshot and resulted to eye infection. Shore medical checkup was arranged by the Master at Fujairah and the OS diagnosed with Corneal Foreign Body and Dry Eye. Foreign body was removed by local anesthesia and recommended to rest for 1 day as per doctor’s advice Type Impact Primary Incident Personnel Severity Impact Secondary Minor
Lessons learnt 1)Incorrect use of equipment- the pressure at the air hose was not released before changing removing the jet chisel. 2)Improper positioning at the task- knowing that rust particles are everywhere in the area and may be present in his body, gloves, boiler suits and the jet chisel itself, the OS should had transfer to a clean area and removed the jet chisel. Since the hose is pressurized with air, he should had moved his face away from the quick connect coupler. 3.) Substandard eye goggle- The goggle is not effective enough to seal the eye & nose area. Message to the Industry 1) New type eye goggle as appended picture or goggles with similar fittings will be provided across the fleet from now on. 2) Continues monitoring shall be done by the C/O, to ensure code of safe working practices were properly carried out, and the control measures identified in the risk assessment was duly complied.
