S. No. 39 Date 08-Mar-2013 Occurrence 39-PILOT LADDERS - In 2012 as per PSA there were 3 incidents where an accident could have happened due poorly maintained Pilot ladders. Investigation revealed degradation of fiber rope and this was at a point which goes through the wooden steps. The ropes had perished due to damp stowage. Pilot ladders were usually stowed on wooden pallets covered by Canvas not allowing the ladder to dry. Type Impact Primary Incident Personnel Severity Impact Secondary Minor Unspecified
Lessons learnt 1) Ship Master's should ensure that Pilot ladders are laid out from time to time and allowed to dry. 2) Before every usage, it is recommended to twist the ropes to check for core disintegration specially in way of wooden steps (batons) Message to the Industry The findings by PSA should be shared with all the installations using rope ladders. The deterioration of inner core could be equally valid for fiber mooring ropes specially in way of the eyes which remain damp and covered under canvas sheath at most times.
