No. 20 Date 08-Feb-2013 Occurrence 20 - PERMIT TO WORK Contractors, including a Safety representative, came onboard and commenced rigging scaffolding over the side without obtaining a ship issued "Permit to Work". The Chief Officer saw what was happening and stopped the work. Type Impact Primary Near Miss Personnel Severity Not applicable Impact Secondary Lessons learnt The Contractors were given a tool-box talk and a Permit to "Work Aloft and Over the side" was issued. Proper procedures were put in place. The contractors were explained why this procedure is important to their own safety and was told to never start work without proper permits ever again. Work resumed in a safe manner. Message to the Industry Another example of empowering the shipstaff to take the right decisions and not to cut corners or sacrifice safety for the sake of commercial gains. NO WORK IS IMPORTANT IF IT CANNOT BE DONE SAFELY. Remarks Good work done by the Chief Officer who also took extra efforts to educate the shore contractors by explaining the Permit to Work and conducting Tool Box Meeting prior approval to commence work.
