KEY MESSAGE: UNMANNED BRIDGE DURING STS? A DEFINITE NO, PLAN RESOURCES.
WHAT HAPPENED:
While STS operation at Mumbai Anchorage, vessel dragged anchor and contacted other anchored vessel.
Two deck officers were engaged in cargo operation in cargo control room due to busy for cargo work, but
there is no continuous anchor watch on bridge. Deck crew realized vessel is dragging but it was too late to
avert contact accident.
WHY IT HAPPENED:
Deck officer should be always on bridge even during STS cargo operation, required to call master or other
officer, or slow down or stop cargo operation as necessary.
LESSONS LEARNED:
1) Need to educate Crew to prioritise safety over commercial matters.
2) Back to Basics – When the vessel is at anchor, position check at regular intervals is crucial. Vessel undergoing
STS is more vulnerable to drag anchor due additional load.
