THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
WHO WILL FOLLOW THIS NOTICE
This Notice describes the health information practices of UAB Medicine Enterprise and certain of its affiliates. All of our hospitals, clinics, physicians, foundations, facilities, and other sites and locations follow the terms of this Notice which may be updated from time to time. The complete list of the entities, sites, and locations can be found on our website at www. uabmedicine. org / legal / notice-of-health-information-practices or may be obtained by contacting the Entity Privacy Coordinator listed at the end of this Notice. In addition, these entities, sites, and locations may share health information with each other for the purposes of treatment, payment, or health care operations as described below.
OUR PLEDGE REGARDING HEALTH INFORMATION
We understand that information about you and your health is personal, and we are committed to protecting your health information. We create a record of the care and services you receive at UAB Medicine Enterprise. We need this record to provide you with quality care and to comply with certain legal requirements. This Notice applies to all the records of your care created by UAB Medicine Enterprise, whether made by clinic / hospital personnel or your personal doctor. This Notice describes the ways in which we may use and disclose your health information and explains your rights and our legal obligations regarding the use and disclosure of your health information. We are required by law to: � make sure that health information that identifies you is kept private; � provide you with this Notice describing our legal duties and privacy practices with respect to your health information; � notify you in the case of a breach of your identifiable health information; and � follow the terms of the Notice currently in effect.
HOW WE MAY USE AND DISCLOSE HEALTH INFORMATION ABOUT YOU
The following categories describe some of the ways that we will use and disclose your health information. Not every use or disclosure in a category will be listed. However, all of the ways we are permitted to use and disclose information will fall within one of the categories.
� Treatment and Treatment Alternatives. We use your health information to provide you with medical treatment or services and may disclose your health information to doctors, nurses, technicians, medical residents or students, or other UAB Medicine Enterprise personnel, as well as to individuals or organizations outside our facilities who are involved in your care. For example, a doctor treating you for a broken leg may need to know if you have diabetes because diabetes may slow the healing process. The doctor may need to tell the dietitian that you have diabetes so that appropriate meals can be arranged for you. Different departments within UAB Medicine Enterprise may share health information about you in order to coordinate services, such as prescriptions, lab work, and x-rays. We may also share information with your local physician, family members, clergy, or others who help provide services that are part of your care after you leave our facilities. We may use and disclose your health information to tell you about or recommend possible treatment options or alternatives that may be of interest to you.
