comfort and security of thei r own homes.

Enrollment:
Individuals who are 18 years of age or older may apply for
enrollment in Fidelis Care at Home if they:
• Are eligible for Medicaid.
• Reside in Albany, Allegany, Bronx, Broome, Cattaraugus,
Cayuga, Chautauqua, Chenango, Columbia, Cortland,
Delaware, Dutchess, Erie, Essex, Fulton, Genesee, Greene,
Hamilton, Herkimer, Kings, Livingston, Monroe, Montgomery,
Nassau, New York, Niagara, Oneida, Onondaga, Ontario,
Orange, Orleans, Oswego, Putnam, Queens, Rensselaer,
Richmond, Rockland, Schenectady, Schoharie, Steuben,
Suffolk, Sullivan, Tioga, Tompkins, Ulster, Warren, Washington,
Wayne, Westchester, or Wyoming counties.
