For many of those needing long term care but not wanting to leave their communities , it can be a challenge to accomplish both at one time .

But that ’ s where Horizon PACE comes in .

“ Horizon PACE coordinates and provides individualized preventive , primary , urgent and longterm care services so

loved ones can continue living in their homes or community ,” according to the website .

There is a Horizon Pace location here in Richmond which serves Madison , Estill , Jackson and Rockcastle Counties .

PACE stands for Program of All-inclusive Care for the Elderly .

The PACE program in Richmond is one of three in the entire state of

Kentucky .

Kentucky is one of the 32 states along with the District of Columbia that has PACE programs , according to the American Council on Aging .

The council also says that PACE programs have proven beneficial for those with Alzheimer ’ s and related dementias .

According to the website for PACE , people as young as 55 can qualify , but the

average age is 76 with multiple , complex medical conditions .

That ’ s not all Horizon does , Horizon has an adult day care where they not only take care of those older than 55 , they also take care of adults older than 21 .

The adult day care help those there take care of medicines and gets them to appoints if needed along with doing activities .
