It was created in response to the 1999 Supreme Court decision to meet the housing needs of persons with serious mental illness who are at risk of institutionalization or those who are transitioning from long-term stays in an institution , such as psychiatric hospitals , personal care homes , and nursing homes . Efforts have been made to move individuals who are capable of living independently from institutions into affordable housing in communities of their choosing . OHI can pay for moving expenses , basic furnishings , and security and utility deposits ; provide an ongoing rental subsidy ; and cover other housing-related expenses that are a barrier to transitioning to the community .
Referrals to OHI can be made by staff of state operated or contracted psychiatric hospitals , CMHCs , or other DBHDID contractors .
The Homeless Prevention Project originated as an attempt to address institutional discharge into homelessness after a survey in Louisville showed that 77.5 percent of persons in homeless shelters had an institutional background . The project offers discharge planning to persons with mental illness who are exiting state-operated institutions , including psychiatric hospitals , the prison system , and foster care programs . The project has an urban and a rural component , with the urban component served by Volunteers of America in Louisville and the rural component served by The Adanta Group in southcentral Kentucky . The goal of the project is to assist potentially homeless persons who are exiting state institutions for return or reentry into the community by linking them to needed community services and supports . These include employment , medical and mental health services , housing , education , social supports and other community-based services , thereby decreasing the number of persons discharged from stateoperated institutions into homelessness .
Permanent Supportive Housing is a Housing First model that helps people with serious mental illness choose , obtain , and keep housing that is decent , safe , affordable , and integrated into the community . Central to the approach is a belief that people with serious mental illness should have the right to live in a home of their own , without any special rules or service requirements . It is also guided by the belief that people need basic necessities like food and a place to live before attending to anything less critical . These programs typically use a case management model to assist clients in locating and applying for a desirable housing placement , filling out all necessary paperwork , and moving them into the unit . Case managers often work with landlords , housing provider agencies , and participants to address issues that may arise in order to ensure stability in housing . Participants are connected to inpatient and outpatient substance use and mental health treatment . With flexible supports , people with serious mental illness can live in housing of their choice , just like any other member of the community .
As a part of the transition out of homelessness , services that provide social skills training to develop and maintain relationships and promote social connections to combat loneliness , social exclusion , and psychological problems play a major role in improving outcomes .
