When you decide to book your Comprehensive Health Assessment( CHA) at one of these new locations, you’ ll receive personalized check-ups and helpful advice to keep you feeling your best.
During your appointment, we’ ll talk about things like how to prevent falls, eat well, stay active, quit smoking, manage your weight, and take care of your emotional well-being. Plus, you’ ll have plenty of time to chat with our caring advanced practice providers about any health concerns you may have. We’ ll even share all the important information from your visit with your primary care provider to make sure they have all the information they need to provide excellent care to you.
