This is your annual chance to enroll, change, or drop coverages outside of a qualifying life event like a birth, adoption, marriage, divorce, etc. 4 Use the materials in your personalized packet, i.e. the cover letter and the Roadmap to Healthcare, to prepare. 4 Each Next Generation Enrollment, Inc. location will have copies of the Summary Plan Description available for you to review. You can also access the Summary Plan Description by clicking here or find it in the Library when you login through www.ngebenefits.com. Details on our plans will be found in this document. 2016 updates to our Summary Plan Description can be found by clicking here. 4 Very Important! The Medical, Dental, Vision and Flexible Spending Plans require an election or you will not have benefits in 2016 under any of these plans. Any of the other voluntary plans that you are currently enrolled in will carry over into 2016. 4 You will enroll in one of two ways: • Online at www.ngebenefits.com (login instructions can be found by clicking here) • Over the phone through the Benefit Center at 888-266-1732 (Click here for how to prepare when enrolling over the phone.) 4 Enroll online 24/7 and the hours of the call center are 8AM to 11PM ET, Monday through Friday. If you plan to enroll over the phone, the closer you get toward the end of the enrollment window, the longer you may wait on hold to speak to a Benefit Center representative. 4 Remember to enroll by December 2 or you will not have medical, dental, vision or FSA benefits in 2016! SECTION 2: Eligible Employees READ MORE Being able to enroll into Next Generation Enrollment, Inc.’s benefit plans differs for Regular Full-time and Regular Part-time employees. SECTION 3: Eligible Dependents Under the Next Generation Enrollment, Inc. benefit plans, you are able to enroll the following types of dependents: 4 Spouse 4 Domestic Partner 4 Dependent Children under the age of 26 (unless disabled) Note: Anytime a new dependent is added to the benefit plans, the employee is required to document the dependent. Should you enroll a dependent during the open enrollment period, the Benefit Center will reach out to you and instruct you on what to do. Please note that proper documentation confirming your dependents and their eligibility for READ coverage will be required by December 31, 2015 for them to have coverage effective January 1, 2016.
