Inflammatory Airway Disease ( Mild / Moderate Equine Asthma )

Disease Definition or the Lack Thereof
For the last 25 years , inflammatory airway disease ( IAD ) has been the all encompassing name for what is probably a collection of respiratory tract disorders which have as a common clinical manifestation the reduction in performance or exercise intolerance , without evidence of infection ( e . g ., fever , leukocytosis ) or increased respiratory effort at rest . Coughing with or without the presence of excess airway mucus is a very common clinical sign . IAD is not a discrete diagnosis in itself as it is usually accompanied by a constellation of non-specific clinical signs . This lack of specificity was emphasized in a consensus statement on IAD published by the American College of Veterinary Internal Medicine in 2007 ( J Vet Intern Med 21:356-361 , 2007 ) and subsequently updated in 2016 ( J Vet Intern Med 30 : 503 – 515 , 2016 ) In the latter publication , the authors proposed that IAD be renamed mildmoderate equine asthma to reflect the similarity of some of its symptoms to those of some human asthma phenotypes . The same paper proposed that Recurrent Airway Obstruction ( RAO ) hitherto be called severe equine asthma . The use of the term asthma with “ mild-moderate ” or “ severe ” as descriptors would reflect the consensus that IAD and RAO are conditions with pathologic processes that are on a common spectrum but are characterized by different clinical severities . Those particularly interested in IAD are encouraged to consult these consensus statements for more information . For the remainder of these notes , the term IAD will be used .
