Question : Can genomic analysis improve clinical care for families after a sudden death in young individuals ? Findings : In this cohort study of 103 decedents from a national collaboration of medical examiners , pathogenic / likely pathogenic variants in arrhythmia or cardiomyopathy genes were identified in 13 %. In multivariate analysis , rare variants in cardiac genes ( pathogenic / likely pathogenic / uncertain significance ) were associated with younger age at death . Meaning : Genomic autopsy found clinically actionable cardiac variants in 13 % of sudden deaths in young individuals , but a more sophisticated model of multiple gene effects or oligogenic model may be required to optimize clinical care for families .

Findings : In this cohort study of 2110 adults with a mean follow-up of 10.8 years , participants taking at least 7000 steps / d , compared with those taking fewer than 7000 steps / d , had a 50 % to 70 % lower risk of mortality . There was no association of step intensity with mortality regardless of adjustment for step volume . Meaning : This cohort study found that higher daily step volume was associated with a lower risk of premature all-cause mortality among Black and White middle-aged women and men .

Findings : In this randomized clinical trial that included 100 patients , treatment with 2 intra-articular platelet-rich plasma injections vs placebo injections with saline resulted in a mean change in the American Orthopaedic Foot and Ankle Society score ( range , 0-100 ; higher scores indicate less pain and better function ) of 10 vs 11 points over 26 weeks ; the between-group difference was not statistically significant . Meaning : These findings do not support the use of platelet-rich plasma injections for patients with ankle osteoarthritis .
