10.3.1.1.1 complete medical records ; 10.3.1.1.2 satisfy threshold eligibility criteria ; 10.3.1.1.3 provide requested information ; 10.3.1.1.4 attend a mandatory meeting ; 10.3.1.1.5 complete or comply with training or educational requirements ;
10.3.1.1.6 comply with a request for fitness for practice evaluation or to execute any required releases ;
10.3.1.1.7 comply with a request for competency assessment or to execute any required releases ; or
10.3.1.1.8 notify the Medical Staff Office of any change in any information on the application form ; or
10.3.1.2 is arrested , charged , indicted , convicted , or pleads guilty or no contest pertaining to any felony or to any misdemeanor involving the following : ( a ) Medicare , Medicaid , or other federal or state governmental or private third-party payer fraud or program abuse ; ( b ) controlled substances ; ( c ) illegal drugs ; ( d ) violent act ; ( e ) sexual misconduct ; ( f ) moral turpitude ; or ( g ) child or elder abuse ; or
10.3.1.3 makes a misstatement or omission on an application form .
10.3.2 Automatic relinquishment will take effect immediately upon actual or special notice to the individual and will continue until the matter is resolved , if applicable .
10.4 Indications and process for precautionary suspension or restriction of clinical privileges
10.4.1 Whenever failure to take action may result in imminent danger to the health and / or safety of any individual , the MEC , Chief of Staff , CEO , CMO , VPMA , or Board Chair is authorized to suspend or restrict all or any portion of an individual ’ s clinical privileges or afford the individual an opportunity to voluntarily refrain from exercising clinical privileges while the matter is being reviewed .
10.4.2 A precautionary suspension is effective immediately and will remain in effect unless it is modified by the CEO and the Board .
10.4.3 Within three days of the imposition of a precautionary suspension , the individual will be provided a brief written description of the reason ( s ) for the precautionary suspension .
