anesthesiologist will perform a pre-anesthesia evaluation , which is sufficient and meets the H & P requirements .
3 . History and Physical examinations completed within thirty ( 30 ) days prior to registration or inpatient admission and are not updated appropriately and consents not appropriately completed prior to the procedure will be considered delinquent and be addressed accordingly . Addendums to the History and Physical attesting to the appropriate update and Consents attesting to what had been completed prior to the procedure will be accepted as addressing but not negating the delinquency .
B . 5 .
All patients undergoing procedures which require an H & P must have an H & P and written update ( if applicable ) on their chart prior to beginning such procedures .

Post procedurally if a patient is anticipated to require Hospital-based services in any status , this should be reflected in the H & P .
Post procedurally if a patient needs to be admitted to the Hospital for Hospital-based services in any status which were not expected prior to the procedure , reason for Hospitalization should be noted either in an update to the original H & P or in a separate progress note within 24 hours of admission .

Except for pathologists , all Medical Staff Members with inpatient privileges , and those granted temporary inpatient privileges may perform and document H & Ps in all inpatient and outpatient settings . All other Medical Staff Members must be granted specific clinical privileges to perform and document H & Ps with the following exceptions :

All category I and II Advanced Practice Providers granted inpatient clinical privileges by the Medical Staff may perform H & Ps unless otherwise specified in their credentialing and / or privileging process .
Medical students and Advance Practice Provider ( APP ) students may perform and document H & Ps under the supervision of a Teaching Attending Medical Staff Member . The Teaching Attending must interview and examine the patient , will be held accountable for the content of such H & Ps and must acknowledge this accountability by countersigning and dating the H & P documentation .
A licensed physician with an MD or DO degree , a licensed Nurse Practitioner , licensed Certified Nurse Midwife , or a licensed Physician Assistant ( with co-signature by the supervising physician ) who are not members of the Medical Staff may provide the H & P required for an
