APPENDIX B : Category 2 ( Traditional )
INITIAL APPLICATION CRITERIA FOR THE DESIGNATION OF APPLICANT FILES
The following criteria define a Category 2 ( Traditional ) application : ( 1 ) The final recommendation is adverse or with limitation ;
( 2 ) The applicant is found to have experienced an involuntary termination of medical staff membership or has a current challenge or a previously successful challenge to licensure or registration ;
( 3 ) Applicant has had two or more malpractice cases filed within the past five years or one final adverse judgment or settlement in a professional liability action in excess of $ 50,000 ;
( 4 ) Applicant changed medical schools or residency programs , except for changes from PGY-1 ( internship ) to a subsequent PGY-2 program or change in specialty , or has gaps in training or practice ;
( 5 ) Applicant has changed practice affiliations more than three times in the past ten years , excluding telemedicine and locum tenens practitioners ;
( 6 ) Applicant has one or more reference responses that raise concerns or questions ;
( 8 ) Discrepancy is found between information received from the applicant and references or verified information ;
( 9 ) Applicant has an adverse National Practitioner Data Bank report related to behavior ;
( 10 ) The request for privileges is not reasonable based upon applicant ’ s experience , training , and demonstrated current competence , and / or is not in compliance with applicable criteria ;
( 11 ) Applicant has potentially relevant physical , mental , and / or emotional health problems ;
( 12 ) Other reasons as determined by a medical staff leader or other representative of the hospital which raise questions about the qualifications , competency , professionalism , or appropriateness of the applicant for membership or privileges .
