( m) imposition of a precautionary suspension;( n) automatic relinquishment of appointment or privileges;( o) denial of a request for a leave of absence or for an extension of a leave;( p) activation of administrative medical leave of absence;( q) removal from the on-call roster or any other reading or rotational panel;( r) decision not to grant, or the withdrawal of, temporary privileges; and( s) termination of any contract with or employment by the Hospital.
8. A. 4. Notice of Recommendation:
The CEO will promptly give special notice of a recommendation which entitles an individual to request a hearing. This notice will contain:
( a) a statement of, and the general reasons for, the recommendation;
( b) a statement that the individual has the right to request a hearing on the recommendation within thirty( 30) days of receipt of this notice; and
( c) a copy of this Article. 8. A. 5. Request for Hearing:
An individual has thirty( 30) days following receipt of the notice to request a hearing. The request must be in writing, to the CEO, and must include the name, address, and telephone number of the individual’ s counsel, if any. Failure to request a hearing will constitute waiver of the right to a hearing, and the recommendation will be transmitted to the Board for final action.
8. A. 6. Notice of Hearing and Statement of Reasons:
( a) The CEO will schedule the hearing and provide, by special notice to the individual requesting the hearing, the following:
( 1) the time, place, and date of the hearing;
( 2) a proposed list of witnesses who will give testimony at the hearing and a brief summary of the anticipated testimony, if known at the time of notification;
