ARTICLE 7 AUTOMATIC ACTIONS
7. A. AUTOMATIC RELINQUISHMENT
( 1) Any of the occurrences described in this Section will constitute grounds for the automatic relinquishment of an individual’ s appointment and clinical privileges. An automatic relinquishment is considered an administrative action, not an adverse professional review action, and, as such, it generally does not trigger an obligation on the part of the Hospital to file a report with the National Practitioner Data Bank.
( 2) Except as otherwise provided below, an automatic relinquishment of appointment and privileges will be effective immediately upon actual or special notice to the individual.
( 3) When a provider becomes subject to the potential need for an automatic suspension as outlined in this article, the Medical Staff Office will confirm the need for suspension and provide notification via email and / or phone call to the provider, clinical leadership as well as key internal stakeholders. This notification will include reason for suspension, start date, end date and how to submit proof of compliance to clear the suspension.
( 4) The Medical Staff Office will communicate the updated status to all applicable departments in the most efficient and timely way possible.
( 5) The provider is only cleared from suspension by the Medical Staff Office when the information is verified, not simply upon the provider’ s submission of the information. The Medical Staff Office will then notify all internal stakeholders notified of the suspension of the provider’ s release from suspension.
( 6) The process described in 7. A. 3-5 will be applicable to all suspension types: automatic, summary and precautionary.
( 7) Items in this section are automatic in nature. Other items requiring Medical Staff committee, Medical Staff leadership or Governing Board approval will be found in those corresponding sections.
7. A. 1. Failure to Complete Medical Records:
Failure of an individual to complete medical records, after notification by the medical records department of the delinquency in accordance with applicable policies and rules and
