( b) If appointment has been granted prior to the discovery of a misstatement or omission, the individual will be informed in writing of the nature of the misstatement or omission and permitted to provide a written response. Failure to respond within fifteen( 15) days may be deemed an immediate and voluntary withdrawal of the medical staff membership and privileges by the provider. The individual will also have an opportunity to meet with the Chair of the Credentials Committee, CMO / VPMA, and Chief of Staff to explain the misstatement or omission. These individuals will review the response and determine what impact, if any, the information has on the member’ s appointment and clinical privileges.
( c) No action taken pursuant to this Section will entitle the applicant or member to a hearing or appeal.
3. B. PROCEDURE FOR INITIAL APPOINTMENT AND PRIVILEGES 3. B. 1. Application Process:
( a) Applications may be provided to residents and fellows who are in the final six months of their training. Final action will not be taken until all applicable threshold eligibility criteria are satisfied.
( b) A completed application form with copies of all required documents must be returned to the Medical Staff Office within thirty( 30) days after receipt. The application must be accompanied by the non-refundable application fee, unless waived by Renown, to be considered complete.
3. B. 2. Initial Review of Application:
( a) As a preliminary step, the application will be reviewed by the Medical Staff Office and the CMO / VPMA, as necessary on an as needed basis, to determine that all questions have been answered. An applicant who fails to return a completed application will be notified that their application will not be processed, until all items are received and only if done within timelines specified as well as being deemed ineligible for the expedited process.
( b) The application will also be reviewed by the Medical Staff Office and the CMO / VPMA, as necessary, to determine that the applicant satisfies threshold eligibility criteria. An applicant who does not satisfy threshold eligibility criteria, nor submit an approved waiver, will be notified that they are ineligible to apply for appointment and clinical privileges and that their application will not be processed. An application may be processed, through initial steps, if the applicant is working toward satisfying threshold criteria( e. g., current license, current DEA registration and state-controlled substance license, or professional liability insurance coverage). Final action will not be taken by the Board until all applicable threshold eligibility criteria are satisfied or the Board has granted a waiver of the criteria.
