Stroke Rehabilitation Program
Our goal is to provide persons served and their caregivers the opportunity to achieve maximum independence and enhance their quality of life. Our experienced interdisciplinary team works together to develop an individualized treatment plan, and with input from persons served, establishes goals for success. This team consists of: physiatrist, rehabilitation nurses, a physical therapist, an occupational therapist, speech therapists, dietitians, pharmacists and case managers.
