INDEPENDENCE OUTCOMES
We measure progress using industry-standard tools for self-care and mobility status upon admission and at discharge to determine the improvement achieved during a stay. Self-care measures, the person’ s ability to perform activities of daily living, include: eating, grooming, personal hygiene, bathing and dressing. Mobility measures are considered activities like: positioning, transferring from different surfaces, walking, building endurance and tackling community skills of different surfaces, curbs, stairs, car transfers and safe fall recovery. Higher scores at discharge show improvement and preparation to go home and return to your community.

Discharge home is our goal and focus for persons served and their caregivers, and 88 % of the people we serve went home compared to 86 % nationally. Seven percent( 7 %) of persons served were discharged to an acute care hospital, and 5.1 % were discharged to a long-term care facility. National comparison is 7 % and 7 % respectively. Persons served at Renown Rehabilitation Hospital achieved discharge self-care scores of 36.9 compared to the national score of 36.3 and discharge mobility scores of 71.6 compared to the national score of 71.2.
