Stroke Rehabilitation Program
Our goal is to provide persons served and their caregivers the opportunity to achieve maximum independence and enhance their quality of life . Our experienced interdisciplinary team works together to develop an individualized treatment plan and , with our persons served input , establishes goals for success . This team consists of : physiatrist , rehabilitation nurses , a physical therapist , an occupational therapist , speech therapists , dietitians , pharmacists and case managers .
