BRAIN INJURY PROGRAM OUTCOMES
The outcome measures include self-care and mobility discharge scores . Self-care measures , the person served ’ s ability to perform activities of daily living include : eating , grooming , personal hygiene , bathing and dressing . Mobility measures are considered activities like : positioning , transferring from different surfaces , walking , building endurance and tackling community skills of different surfaces , curbs , stairs , car transfers and safe fall recovery . Higher scores upon discharge show more improvement and preparation to go home and return to your community . Persons served at Renown Rehabilitation Hospital achieved discharge selfcare scores of 34.6 and discharge mobility scores of 66.7 .

WHERE DO THE PEOPLE WE SERVE GO AFTER REHABILITATION ?
Discharging home is our goal and focus for the person served and their caregivers , 78 % of the people we served went home . Brain injuries are complex . Fourteen percent ( 14 %) of persons served needed to return for additional acute medical care compared to 10 % for region and nation . Not everyone is safe going home or has adequate support to go home . Seven percent ( 7 %) were discharged to a long-term care facility as compared to region at 6 % and nation 7 %.
