Written Notification
The Dental Law and State Board of Dentistry regulations require that there are certain instances in which a provider is required to notify the state board in writing. Here is a list of those instances and where they are located.

Section 11.2( b)( 2) Nitrous Calibration
Applicants for restricted II permits( nitrous oxide / oxygen analgesia) must provide the board with the make, model and serial number of any equipment utilized by the applicant, and certification that the equipment is in proper working order. Permit holders must provide evidence that that their equipment is properly calibrated to the board at least once every six years.
Section 11.3 Reports of Adverse Occurrences
Licensees are required to notify the board in writing within 30 days of any occurrences of death or unusual incidents requiring medical care, that result in either physical or mental injury of a patient, as a direct result of the administration of either anesthesia or drugs.
Any occurrences of death not related to drugs or anesthesia must also be reported within 30 days.
Section 11.5 Reporting of Multiple Licensure or Certification
Any licensee who is also licensed in any other state, territory or country should report this information to the board on their biennial renewal application. Any disciplinary action that is taken in other states, territories or countries should be reported to the board on the biennial renewal application, or within 90 days of disposition, whichever comes first.
